Patient decrees
With a patient decree, you have the opportunity to make your own decision on medical decisions today. This is particularly important in the event that you are no longer able to communicate your wishes yourself due to an accident or illness.

A patient decree gives your family and the medical team which is providing care the certainty that they can act in your best interests in an emergency.
Would you prefer to use printed brochures and templates to prepare your care documents? The Caritas Care Folder can help you with this. The folder contains information brochures, patient decree forms and a guide to drawing up your advance care directive and your will, as well as a document for directives in the event of death.
We will be happy to send you the Care Folder in the post in return for a small contribution towards the costs of production and postage.
