Putting your own wishes in writing

This guide gives you the opportunity to put your wishes in writing for questions that will arise after your death. We recommend that you discuss these directives with your relatives and inform them about their existence.

The Caritas Care Folder: guidelines and templates

The folder contains information brochures, patient decree forms and a guide to drawing up your advance care directive and your will, as well as a document for directives in the event of death.
